Background Scientific and technological advancements in mapping and understanding the interrelated pathways through which biological and environmental exposures affect disease development create new possibilities for detecting disease risk factors. Early detection of such risk factors may help prevent disease onset or moderate the disease course, thereby decreasing associated disease burden, morbidity, and mortality. However, the ethical implications of screening for disease risk factors are unclear and the current literature provides a fragmented and case-by-case picture. Methods To identify key ethical considerations (...) arising from the early detection of disease risk factors, we performed a systematic scoping review. The Scopus, Embase, and Philosopher’s Index databases were searched for peer-reviewed, academic records, which were included if they were written in English or Dutch and concerned the ethics of (1) early detection of (2) disease risk factors for (3) disease caused by environmental factors or gene-environment interactions. All records were reviewed independently by at least two researchers. Results After screening 2034 titles and abstracts, and 112 full papers, 55 articles were included in the thematic synthesis of the results. We identified eight common ethical themes: (1) Reliability and uncertainty in early detection, (2) autonomy, (3) privacy, (4) beneficence and non-maleficence, (5) downstream burdens on others, (6) responsibility, (7) justice, and (8) medicalization and conceptual disruption. We identified several gaps in the literature, including a relative scarcity of research on ethical considerations associated with environmental preventive health interventions, a dearth of practical suggestions on how to address expressed concerns about overestimating health capacities, and a lack of insights into preventing undue attribution of health responsibility to individuals. Conclusions The ethical concerns arising with the early detection of risk factors are often interrelated and complex. Comprehensive ethical analyses are needed that are better embedded in normative frameworks and also assess and weigh the expected benefits of early risk factor detection. Such research is necessary for developing and implementing responsible and fair preventive health policies. (shrink)

This paper provides new elements for reconstructing Heinrich Hertz's conversion from an electrodynamic concept to a concept of field theory, a process that took place between September 1887 and February 1888. First of all, it is argued that, contrary to what one could deduce from Hertz's own presentation in his Untersuchungen (or Electric Waves), paper N∘ 5 was actually written after the first publication of papers N∘ 6 and N∘ 7; this fact is illuminated by Hertz's unpublished correspondence (Section 1). (...) Following this reordering, an approximative process of Hertz's conversion based only on his published papers is described (Section 2). Second, by complementing Hertz's published Erinnerungen (or Memoirs, Letters, Diaries) with his unpublished Versuchsprotokolle (Laboratory notes), a much finer description of this conversion process is discussed (Section 3). It is finally concluded that Hertz's idea of “air waves” belongs at the very end of this process, and was mainly attained through experimental hints found within Helmholtz's theoretical framework. ExcerptIn 2021, in the pages of this journal, I contended that a coalition of interests in the United States had coalesced in opposition to the presidency of Donald Trump and duly taken power through the vehicle of Joe Biden.1 This coalition includes the Democratic Party, corporate elites, the media, academia, and—the subject of the present article—the national security (natsec) state. In that earlier piece, I focused on particular components of this coalition: legacy and social media. I went on in a (...) subsequent Telos piece to examine another of its elements: the academy.2 These fractions all appear, however, to be relatively minor players. While I argue that the legacy media have staged a counterrevolution to seize back control of the public sphere, this exhausts their parochial, pecuniary agenda; beyond this, they are simply part of a wider activist liberal cause. Contemporary liberal activism is in turn ideologically rooted in ideas propagated by sectors of the academy and hegemonic within it, but this has not, on my analysis, empowered the academy so much as the forces that make it increasingly redundant. In this article, I consider the interoperation of twin contemporary governmental imperatives, fostering economic growth and ensuring biopolitical security, in the face of the COVID-19 pandemic. At a theoretical level, I thereby consider the question of the applicability of a Marxist analysis vis-à-vis a Foucauldian one in understanding state responses to the pandemic. Despite the apparent prioritization of preserving life over economic activity by governments around the world in this context, I will argue that the basic problem that COVID-19 posed (...) for the state was one of sheer unknowability and that the fundamental motive for the governmental response was a concern for security in Foucault’s sense, that is, ensuring a baseline predictability in the social field, upon which economic activity, like myriad other social activity, is premised. I argue that this drive for security motivated states to appeal to medical experts to determine the direction of their response, who in turn applied a default model of quarantine. While we cannot be certain that the medically-guided response was optimal in terms either of health outcomes or economically, I argue it served its essential purpose by providing a structured framework for social action in the face of the unknown. While this is vital for the maintenance of the basic coordinates of capitalist society, I argue it nonetheless cannot entirely be explained simply by an appeal to Marxist categories and instead requires Foucault’s insights into the medicalization of society. In this review essay, I discuss how Lambert Zuidervaart relates to the philosophy of Herman Dooyeweerd in two of his books. He himself calls his work in this regard a critical retrieval. To answer the question as to how far this characterization is justified, we will look at Zuidervaart’s discussion of the religious antithesis and the transcendental critique, his idea of macrostructures and societal evil, and his view of truth and knowledge.

To incorporate medical ethics into clinical practice, it must first be understood and valued by health care professionals. The recognition of this principle led to an expanding and continuing educational effort by the ethics committee of the Vancouver General Hospital. This paper reviews this venture, including some pitfalls and failures, as well as successes. Although we began with consultants, it quickly became apparent that education in medical ethics must reach all health care professionals—and medical students as well. Our greatest successes (...) came in the formative years of a medical career (ie, in medical school and residency training programmes), but other efforts were not wasted, particularly among nurses and other health care professionals.Although this is a personal review of the experience in one institution, the lessons learnt in Vancouver are applicable to the further development of medical ethics in the UK. In social identity theory, the in-Christ identity is understood as primarily a socially directed process in which people categorise themselves relative to other groups. Intergroup behaviour would cause them to discriminate against the so-called ‘outgroup’, favouring the so-called ‘ingroup’. Although social identity complexity theory has moved beyond single ingroup-outgroup categorisation, it is a question if social identity theories can fully account for the in-Christ identity, especially within an African context. In African religious identity, identity is linked to both the community (...) and the ruling deities, which are perceived to be real entities that intervene in human affairs and identity, presupposing a supernaturalistic epistemology. Contribution: In this contribution, the naturalistic epistemological underpinnings of social identity theories are reconsidered in light of a supernatural aspect that is argued to be constitutive of the 1st century in-Christ identity as well as the African Christian identity. Amid current approaches to the in-Christ identity, which is mainly a Pauline concept, some of the main Pauline texts that have bearing on the in-Christ identity are revisited in light of current theories and epistemologies on identity. The reception history of the French Revolution in France and England is well documented among Anglophone scholars; however, the debate over the Revolution in Germany is much less well known. Fichte's Contribution played an important role in this debate. Presented here for the first time in English, Fichte's work provides a distinctive synthesis of Locke's "possessive individualism," Rousseau's general will, and Kant's moral philosophy. This eclectic blend results in an unusual rights theory that at times veers close to a form (...) of anarchism. Written in 1792–93, just before Fichte moved to Jena to develop his philosophical system in a series of works—above all the Wissenschaftslehre of 1794—the Contribution provides invaluable insight into Fichte's early development. ABSTRACT This article explores the relationship between rhetoric and violence by running this pairing through a corresponding couplet: rhetoric and race. Arguing for a common substrate between these two pairs of terms—coloniality—this article proposes that rhetorics of “nonviolence” are better understood as rhetorics of anti-violence.

Digital transformation and “artificial intelligence (AI)”—which can more adequately be called “data-based systems (DS)”—comprise ethical opportunities and risks. Therefore, it is necessary to identify precisely ethical opportunities and risks in order to be able to benefit sustainably from the opportunities and to master the risks. The UN General Assembly has recently adopted a resolution aiming for ‘safe, secure and trustworthy artificial intelligence systems’. It is now urgent to implement and build on the UN General Assembly Resolution. Allowing humans and the (...) planet to flourish sustainably in peace and guaranteeing globally that human dignity is respected not only offline but also online, in the digital sphere, and in the domain of DS, requires two policy measures: (1) human rights-based data-based systems (HRBDS): HRBDS means that human rights serve as the basis of digital transformation and DS. (2) International Data-Based Systems Agency (IDA): IDA should be established at the UN as a platform for cooperation in the field of digital transformation and DS, fostering human rights, security, and peaceful uses of DS, as well as a global supervisory institution and regulatory authority in digital transformation and DS. The establishment of IDA is realistic because humanity has already shown in its past that we are able to not always “blindly” pursue the technically possible but also to limit ourselves to what is technically feasible when humanity and the planet are at stake. For instance, humans researched the field of nuclear technology, developed the atomic bomb, and detonated it several times. Nonetheless, the same humans limited research and development in the field of nuclear technology to prevent even worse consequences by establishing the International Atomic Energy Agency (IAEA) at the UN. Unfortunately, today many scholars and general readers neglect the work of the Fathers of the Church. One often hears that the writings of the Fathers have become less relevant or less important because there has been so much progress in Scripture studies and because the Fathers did not really try to find out what the inspired writer meant to say-for they relied on allegory. St. Augustine's exegesis poses a particular difficulty in terms of its reliance on allegory. Augustine was strongly influenced by the allegorical exegesis of bishop Ambrose and recounts in his Confession.5 (VI, iv, 6) how he used to hear Ambrose teaching in his sermons, as though he were most diligently teaching a rule: "The letter kills, but the spirit gives life." Father de Margerie admits that Augustine did indeed suffer from "excessive allegorism" (126), but he insists that this is no reason to give up entirely on Augustine. Instead, with keen scholarly analysis he shows that, in spite of some defects, Augustine did in fact give us some really fine work in Scripture study. Father de Margerie stresses that according to Augustine the purpose of all Scripture is to teach love: "... the plenitude and the end of the Law and of BOOK REVIEWS 507 all the sacred Scriptures is the love... of a Being which is to be enjoyed and of a being that can share that enjoyment with us" (20; from De Doctrina Christiana, I, xxv, 39). This love is taught even at the beginning of Scripture: "... on a subjective level, Moses wrote for the love of all men" (30). But how could this be? "It seems to us nowadays that Augustine failed to perceive adequately the progressive nature of the disclosure of religious truths by the God who revealed them" (30). Father de Margerie responds to this objection by pointing out that given the belief that Moses had been given a transient, "face to face vision of the Divine being... and that this experience was granted him precisely in favor of the universal Church," it makes sense to assume that "the prophet and maker of the old Law received knowledge, during this vision and through divine revelation, of how future interpretation of the Law he was issuing-like that which Jesus was to issuewould extend even to enemies the love of one's neighbor" (31). Augustine was led to this view in the light of the varied interpretations being proposed in his own time for Genesis (48). Father de Margerie admits that "Augustine himself limits the scope of his theory concerning the plurality of literal meaning to a few rather exceptional verses of scripture" (65). In light of modern historical experience, we can see the onesidedness of conceptions according to which the types and directions of activity are mechanically predetermined by conditions external to it, and we can see the necessity of understanding the laws of activity itself in all their complicated dialectical essence. The state-of-art research in the field of life’s organization confronts the need to investigate a number of interacting components, their properties and conditions of sustainable behaviour within a natural system. In biology, ecology and life sciences, the performance of such stable system is usually related to homeostasis, a property of the system to actively regulate its state within a certain allowable limits. In our previous work, we proposed a deterministic model for systems’ homeostasis. The model was based on dynamical system’s (...) theory and pairwise relationships of competition, amensalism and antagonism taken from theoretical biology and ecology. However, the present paper proposes a different dimension to our previous results based on the same model. In this paper, we introduce the influence of inter-component relationships in a system, wherein the impact is characterized by direction (neutral, positive, or negative) as well as its (absolute) value, or strength. This makes the model stochastic which, in our opinion, is more consistent with real-world elements affected by various random factors. The case study includes two examples from areas of hydrobiology and medicine. The models acquired for these cases enabled us to propose a convincing explanation for corresponding phenomena identified by different types of natural systems. (shrink)

This paper explores ethical issues relating to the management of patients who are terminally ill and unable to maintain their own nutrition and hydration. A policy of sedation without hydration or nutrition is used in palliative medicine under certain circumstances. The author argues that this policy is dangerous, medically, ethically and legally, and can be disturbing for relatives. The role of the family in management is discussed. This issue requires wide debate by the public and the profession.
